Starting in the 2018-19 academic year, we have changed to our 2 week “Mini-Block” schedule that allows more frequent but shorter blocks of inpatient rotations and a separation of inpatient training from outpatient continuity clinic. Block scheduling is an entirely different way to structure a residency program. In the traditional structure residents have a 1-3 half days of continuity clinic during their inpatient rotations. In this structure, residents often feel torn between their inpatient and outpatient duties, and they rarely feel focused on their ambulatory training. Under this “new & improved” scheduling system, our residents are immersed in their continuity clinic early in residency, which enables them to build a strong foundation in ambulatory skills earlier in training. Additionally, removal of clinics from inpatient rotations helps keep the whole team present each day, allowing for better continuity with patients, fewer hand offs, and more time for teaching and learning. While we are formally evaluating the impact of this new schedule on resident learning, continuity of care, patient access, and overall volume of clinical training, thus far our residents & faculty have given this new schedule a big “thumbs up.”
